Managing Heavy Incontince

Struggling to keep your husband dry despite briefs and pads? Double briefing may be causing leaks. High-absorbency briefs with booster pads or a Texas (external) catheter can help keep him dry and comfortable—especially overnight.

Heavy wetters and incontinence products

No matter how many pads and waterproof sheets I use, I can’t seem to keep my husbands bed dry. I use two pairs of briefs at a time. He still pees through. Any suggestions?

Hi Joyce,

Thank you so much for reaching out—what you’re going through is incredibly frustrating, and it’s clear you’re doing everything you can to keep your husband comfortable and dry.

When even layering briefs and using multiple waterproof pads isn’t working, it may be time to consider some alternatives that can offer better protection and reduce the constant cleanup.

Here are some suggestions that may help:

  1. Avoid Double Briefing:
It might seem helpful to double up on briefs, but this can actually reduce absorbency. Adult briefs are designed to absorb a certain amount, and wearing two at once can cause leaks because the moisture doesn't transfer effectively to the second brief. Instead, try using a high-capacity brief with a booster pad inside.
    • Booster pads (like NorthShore EternaDry or Tranquility TopLiner) are designed to be placed inside briefs and allow liquid to pass through to the brief once they're full. This combo can significantly increase capacity without the risk of leaking.
  2. Upgrade to a High-Absorbency Brief:
Look for overnight briefs specifically made for heavy incontinence. Some of the best options include:
    • NorthShore Supreme or MEGAMAX
    • Tranquility ATN (All-Through-the-Night)
    • Abena Abri-Form Level 4
  3. Consider a Texas (External Condom) Catheter:
Since your husband is a heavy wetter, an external catheter may provide a solution that keeps him completely dry and reduces the risk of skin breakdown or infection. A Texas catheter is worn like a condom and connected to a drainage bag. It's non-invasive and can be very effective, especially overnight.
    • You'll want to ensure proper sizing and skin prep (using a skin-friendly adhesive or barrier wipe).
    • It may take some trial and error to find the right brand and fit, but many caregivers find this to be a game-changer.
  4. Protect the Bed More Effectively:
    • Use a zippered waterproof mattress cover underneath the sheets for full protection.
    • Place two washable bed pads (one on top of the other) across the middle of the bed for faster changes.
    • Some caregivers rotate between two sets of pads and sheets to make nighttime cleanup easier.
  5. Talk to a Healthcare Provider:
If leaks continue or you’re concerned about skin irritation or infection, it’s worth discussing these issues with his doctor or a urologist. They may recommend options like a catheter for overnight use or medication to help manage bladder function.

Joyce, you're doing such a loving and difficult job. You're not alone in this, and there are ways to improve the situation. If you'd like help choosing specific products or learning how to apply a Texas catheter properly, I’d be happy to walk you through that as well.
