Preventing Falls Checklist for Older Adults
Are you a Fall Risk?

Use this home fall-prevention checklist to review common hazards with an older adult or family caregiver. The downloadable PDF covers stairs and steps, floors, kitchens, bedrooms, and bathrooms.

Start with these representative checks from CDC STEADI guidance:

  • Keep stairs and walking paths clear of objects.
  • Remove loose rugs or secure them with nonslip backing.
  • Make sure lights are easy to reach, including near the bed and on stairs.
  • Have sturdy handrails and properly installed bathroom grab bars where support is needed.

Revisit the home after a fall, a move, or a change in vision, mobility, or medicines. Ask the older adult what feels difficult, and discuss individual risks with a clinician, pharmacist, vision professional, or occupational or physical therapist as appropriate.
