Use this as a conversation tool, not a test. The goal is a routine that fits the person's habits, preferences, energy, and support needs.

How to use this schedule

  • Complete it with the person whenever possible.
  • Start with a few dependable anchors, such as waking, meals, rest, and bedtime.
  • Add the details that make each part of the day easier, including preferred places, helpful prompts, and calming approaches.
  • Keep the finished schedule where the care team can see it and update it when needs change.

Plan the parts of the day that matter most

A useful plan includes more than tasks. Note the person's preferred foods, the pace of personal care, meaningful activities, quiet time, and the people they enjoy connecting with. Offer choices whenever it is safe and practical.

Meals and hydration

Record usual meal times, preferred settings, foods to keep available, and any reminders the care team should follow. For medical or dietary questions, use the guidance of a qualified professional.

Personal care, activity, and rest

Write down the order that feels most comfortable for dressing and hygiene, the person tends to have the most energy, and which activities or breaks help the day go well.

Make the routine easy to share

Use the notes area to capture communication preferences, mobility reminders, sensory needs, or changes that the next caregiver should know. A short, current note is more useful than a perfect plan that no one sees.
